If a death has just occurred, please call us directly at (603) 532-6484 to report the death to us
before submitting information via this online planner.
If you are not comfortable entering information here online, please download and print our Memorial Planning Questionnaire. This questionnaire can also simply help you organize and gather the information we will need. Please bring it with you, along with all additional documentation, photos, and clothing items to our scheduled in-person appointment.
